The Willow Park Health Primary Care Nurse Practitioner Fellowship Program is a structured, paid training program designed to support newly graduated and early-career Primary Care Nurse Practitioners as they transition into confident, independent practice—while delivering onsite primary care to older adults in assisted living and senior living communities.
This program was intentionally built to replace the traditional “sink-or-swim” model with mentorship, supervision, and sustainable clinical growth

What You Will Do

As a Primary Care NP Fellow, you will provide onsite primary care to seniors residing in:
-
Assisted Living
-
Independent Living
-
Memory Care (primary care support & coordination)
You’ll gain hands-on experience managing:
-
Chronic conditions (HTN, DM, CHF, COPD, CKD, CAD)
-
Polypharmacy & medication reconciliation
-
Preventive and geriatric screening
-
Acute issues managed onsite (UTIs, dehydration, edema, URI, dizziness, wounds)
-
Falls risk, mobility issues, and functional decline
-
Care coordination with families and facility staff

Onsite primary care visits in senior living communities, Scheduled supervision and mentorship, Weekly didactics and QI sessions, Time for documentation and clinical reflection
