The Willow Park Health Primary Care Nurse Practitioner Fellowship Program is a structured, paid training program designed to support newly graduated and early-career Primary Care Nurse Practitioners as they transition into confident, independent practice—while delivering onsite primary care to older adults in assisted living and senior living communities.

This program was intentionally built to replace the traditional “sink-or-swim” model with mentorship, supervision, and sustainable clinical growth

Why This Fellowship Exists

Primary care for older adults is deeply rewarding—but clinically complex.

New NPs often face:

  • High patient complexity without adequate support

  • Productivity pressure before clinical confidence develops

  • Limited exposure to geriatric-specific care models

  • Professional isolation early in practice

What You Will Do

Nurse Assisting Patient

As a Primary Care NP Fellow, you will provide onsite primary care to seniors residing in:

  • Assisted Living

  • Independent Living

  • Memory Care (primary care support & coordination)

You’ll gain hands-on experience managing:

  • Chronic conditions (HTN, DM, CHF, COPD, CKD, CAD)

  • Polypharmacy & medication reconciliation

  • Preventive and geriatric screening

  • Acute issues managed onsite (UTIs, dehydration, edema, URI, dizziness, wounds)

  • Falls risk, mobility issues, and functional decline

  • Care coordination with families and facility staff

Geriatric-Focused Primary Care

This is not traditional outpatient primary care. You’ll learn how to deliver high-quality, relationship-based care directly within senior living communities.

Structured Supervision & Mentorship

Supervision is foundational—not an afterthought.

  • Regular 1:1 supervision

  • Case review and clinical guidance

  • Access to experienced primary care clinicians and physician collaborators
    You are never practicing alone

Gradual Patient Panel Growth

Your panel grows intentionally over time—allowing you to:

  • Develop strong clinical reasoning

  • Build confidence without productivity pressure

  • Form meaningful, longitudinal patient relationships

Ongoing Education & Quality Improvement

Weekly didactics and QI participation focused on:

  • Geriatric primary care best practices

  • Transitional care & hospital avoidance

  • Medication safety and deprescribing

  • Interdisciplinary collaboration in assisted living

What a Typical Week Looks Like

​Onsite primary care visits in senior living communities, Scheduled supervision and mentorship, Weekly didactics and QI sessions, Time for documentation and clinical reflection

Collaboration with nursing, administrative, and care teams

Compensation and Structure

This is a paid fellowship.

Fellows receive:

  • Competitive compensation

  • W2 or 1099 option

  • Reduced patient panel aligned with training goals

Upon successful completion, fellows may transition into ongoing Primary Care NP roles with increased compensation and continued mentorship.
